The major focus of the proposed project is the treatment of menopausal hot flashes and the effects of these interventions upon laboratoryrecorded sleep Within this clinical trial we propose to examine some of the mechanisms of these treatments using analyses of heart rate variability and to provide validation data for a newlyinvented ambulatory hot flash recorder. My interest in sleep research stems from my studies with Dr. Alan Rechtschaffen at the University of Chicago I then applied behavioral treatments to a highly prevalent sleep disorder insomnia in my doctoral dissertation Freedman and Papsdorf 1976 I became interested in the physiology and treatment of menopausal hot flashes at the suggestion of a graduate student Germaine and Freedman 1984 and have pursued these topic areas through 22 years of funding from NIA 10 of these as a Merit Award – AG05233 During this period I expanded my laboratory from a staff of 2 to a staff of 15 and collaborated with many scientists in other areas such as biochemistry genetics cardiology and pulmonology This has required considerable administrative and leadership skills. I have further developed these skills during my terms as Editor of 3 scientific journals: Biofeedback and SelfRegulation The Open Womens Health Journal and The Open Journal of Biomedical Devices which have broadened my scope of scientific knowledge and increased my sensitivity to issues of budget and of compromise I have had considerable experience in the conduct of large multisite clinical trials in my roles as PI or CoPI of the Raynauds Treatment Study RTS Study Group 2000 The Womens Health Initiative and several industrysponsored pharmacologic studies. I am a founding member of the North American Menopause Society 1989 and have been a member of the Board of Trustees from 2004 to 2010.For the above reasons I am wellqualified to be the PI of the proposed project.

Biography :

Dr. Kovac is currently an Emeritus Distinguished Professor of Gynecologic Surgery at Emory. He received his BA degree from Washington University in St. Louis, Missouri, and his MD degree at the University of Missouri. He completed a rotating internship at Northwestern University, and residency in obstetrics and gynecology at Barnes Hospital in St. Louis. He served as a Major in the United States Army and was a General Surgeon and Co-Hospital Commander in Viet Nam. He was awarded the Bronze Star, the Combat Medic Badge, and the Viet Nam Paratrooper Wings. He returned stateside and completed Fellowship in Gynecologic Surgery and Urogynecology at Barnes Hospital. He began his academic career at Washington University School of Medicine as a Clinical Assistant Professor of Obstetrics and Gynecology. He was a member of the Teaching Staff at St. John’s Mercy Medical Center , St. Louis, 1979-1982 and was named Director of the Continence Clinic at Miami Valley Hospital, Dayton Ohio, in 1995. Dr. Kovac was Professor and Director, Division of Pelvic Reconstructive Surgery and Urogynecology and Division of General Obstetrics and Gynecologic Surgery, Wright State University School of Medicine, Department of Gynecology and Obstetrics from 1994 to 2000. He joined the Department of Gynecology and Obstetrics, Emory University School of Medicine, in 2000. Dr. Kovac is a Diplomate of the American Board of Obstetrics and Gynecology; Fellow of the American College of Obstetricians and Gynecologists; Fellow, American College of Surgeons; and Fellow, American Academy of Cosmetic Surgery. He is a member of the Society of Gynecologic Surgeons, Society of Pelvic Reconstructive Surgeons (President), American Urogynecological Society, American Society for Colposcopy and Cervical Pathology, American Society of Gynecologic Laparoscopists, American Medical Association, Association of Professors of Gynecology and Obstetrics, and the Southern Medical Association. Dr. Kovac was the Director of the University of Missouri Medical School Foundation and he is Founder and Director of the International Vaginal Surgery Conferences. Dr. Kovac has been a consultant and has served on numerous committees including Council on Resident Education in Obstetrics and Gynecology, “Educational Objectives,” 1986; Veterans Administration and United States Air Force Ob/Gyn Consultant, Pelvic Reconstructive Surgery and Urogynecology; Clerkship Curriculum Committee and Resident Education and Resident Evaluation Committees, Department of Gynecology and Obstetrics, Wright State University; and Medical Education Committee (Director) and Continuing Medical Education, Miami Valley Hospital. He has served as an Editorial Consultant for Obstetrics and Gynecology, American Journal of Obstetrics and Gynecology, Journal of Pelvic Surgery, and International Journal of Obstetrics and Gynecology. Dr. Kovac has been a guest faculty member at many postgraduate conferences and he has written over 80 journal articles, chapters and books. Dr. Danny Schust is an Associate Professor and the Director of Reproductive Endocrinology and Fertility at The University of Missouri School of Medicine in Columbia, MO, USA. He completed his residency in obstetrics and gynecology at The University of North Carolina at Chapel Hill, his fellowship in Reproductive Endocrinology at Brigham and Women’s Hospital, Harvard Medical School, and post-doctoral basic science training at The Massachusetts Institute of Technology. Dr Schust joined the staff at The University of Missouri in 2006. His clinical interests include general infertility, recurrent pregnancy loss, oncofertility and pediatric and adolescent gynecology. Dr. Hugh S. Taylor is Professor of Obstetrics, Gynecology and Reproductive Sciences, and Director of Reproductive Endocrinology and Infertility at the Yale University School of Medicine. He is also Professor of Molecular, Cellular and Developmental biology at Yale University. He is a board certified specialist in Obstetrics/Gynecology and in Reproductive Endocrinology. Dr. Taylor received his undergraduate training at Yale University and received his medical degree from the University of Connecticut School of Medicine. He completed his residency in Obstetrics and Gynecology at Yale. His postdoctoral training included a fellowship in Reproductive Endocrinology and Infertility as well as a fellowship in Molecular Biology, both at Yale.Dr. Taylor is a recipient of eight National Institutes of Health research grants and directs The Yale Center for Reproductive Biology. His clinical research centers on implantation, endometriosis and menopause. His basic science research focuses on uterine development, the regulation of developmental gene expression by sex steroids, endocrine disruption, and on stem cells. Dr. Taylor has published more than 125 articles and in leading medical journals. He is the Editor-In-Chief of Reproductive Sciences. He serves on several editorial boards and as a reviewer for numerous scientific journals and is frequently invited as a speaker at national and international medical meetings. Dr. Taylor has received numerous awards including the National Institutes of Health Physicians Scientist Award and the Society for Gynecologic Investigation President’s Achievement Award. He has also served as the academic mentor of numerous trainees and has twelve times been awarded the Society of Gynecologic Investigation President’s Presenter Award for this training.

Dr. Pamela D. Berens is a Professor of OB/GYN at the University of Texas, Houston. Clinically, She is a generalist OB/GYN with surgical interests in laparoscopic and robotic procedures. I participate in education in OB/GYN and breast health and chair the OB/GYN quality improvement committee. Dr. Natalie Rasgon is a Professor in the Department of Psychiatry and Behavioral Sciences and Obstetrics and Gynecology in the Stanford School of Medicine. Dr. Rasgon has been involved in longitudinal placebocontrolled neuroendocrine studies for two decades. She has been involved in neuroendocrine and brain imaging studies of estrogen effects on depresxed and menopausal women for many years. Dr. Rasgon is also the director of the Behavioral Neuroendocrinology Program and Women’s Wellness Program in the Department of Psychiatry and she also founded the Behavioral Neuroendocrinology program which focuses on the interaction between reproductive hormones and brain function.

Dr. Enrique Hernandez received his MD in 1977 from the University of Puerto Rico School Of Medicine. Dr. Nazs research interests are in the area of testis-specific gene expression, contraceptive vaccine development, fertilization, and infertility. His laboratory focuses on delineating sperm-specific antigens that have a role in antisperm antibody-mediated infertility, and can be used for the development of contraceptive vaccines for men and women. Various advanced techniques of molecular biology, immunology, cell biology, reproductive biology, proteomics, and genomics are employed. Over ten genes that are specifically expressed in human testis, ovary, and prostate have been cloned and sequenced in the laboratory. Novel genes are actively being searched that have a role in gamete (sperm/oocyte) function and signal transduction pathways. Recombinant, DNA, and peptide vaccines are developed based upon these novel genes/recombinant proteins/bioactive peptide epitopes for contraceptive vaccine development.

Dr. Gil Mor is an Professor Tenure of Obstetrics and Gynecology and Reproductive Science at Yale University School of Medicine. Dr. Mor has focused his studies on the interaction between reproductive organs and the innate immune system. In his research he examines topics related to the immunology of implantation, the role of apoptosis in tissue remodeling and cancer, as well as the role of inflammation in cancer formation and progression. Dr. Mor earned his Ph.D. in Immunoendocrinology from the Weizmann Institute of Science and his M.S. and M.D. from Hebrew University in Jerusalem. He completed postgraduate training at the National Institutes of Health where he worked in the Laboratory of Immunology as a Postdoctoral Fellow. At Yale University, he serves as Director of the Reproductive Immunology Unit and the Translational Research Program, Discovery To Cure DTC. The Reproductive Immunology Unit has made important contributions in our understanding of the immunology of the female reproductive tract in normal and abnormal conditions. His studies on apoptosis and cancer, primarily the role of the Fas/FasL system in tissue renewal and the regulation of apoptosis in cancer cells and immune cells, have been published in prestigious journals and are highly cited. Further, Dr. Mors studies on the regulatory pathways controlling cell survival and apoptosis in ovarian cancer cells led to the characterization of a new compound, phenoxodiol, which was used as a chemosensitizer based on its capacity to reactivate the apoptotic cascade. In 2005, Dr. Mors laboratory developed and characterized a new test for the early detection of ovarian cancer.The test evaluated likelihood of developing ovarian cancer among high risk populations.This test has been licensed to several companies in the USA, China, Europe and Israel. More recently, Dr. Mors laboratory identified a unique population of ovarian cancer cells with characteristics of stem cells.These findings have opened a new area of research which may help to understand the source of recurrence and chemoresistance.Dr. Mor has more than 170 publications and is the editor of two books on Immunology of pregnancy and Apoptosis and Cancer.Dr. Mor is recipient of several national and international prizes, including the J. Christian Herr Awardfrom the Society for Reproductive Immunology. He is member of the American Association for Cancer Research and the Society for Gynecologic Investigation and is the Treasurer of the American Society of Reproductive Immunology. He is also honorary member of several scientific societies in South America. Dr. Mor was elected as the Editor in Chief of the Journal of the American Society for Reproductive Immunology and is the member of the Editorial Board of numerous journals. Dr. Mor has three patents licensed to pharmaceutical companies.Dr. Mor is also founder and organizer of the DTC High School internship program; an educational program to promote science in high school students, undergraduate students, and science educators. The program is an internationally recognized educational activity, enrolling 30 students out of more than 200 applications per year. The program is supported by NIH/NCI and private foundations.

Dr. Tekmal received his PhD in Biochemistry from the N.D.R.I./Kurukshetra University, India, in 1982. He was a faculty member of this department from 1989-1995. In June 1995, Dr. Tekmal moved to Emory University, where he rose to the rank of Professor. Dr. Coney is the dean of the medical school and also holds the title of senior vice president for health affairs at Meharry. Despite her busy schedule, she continues to teach classes and maintain a small practice. "I still see patients every week," she told CBB. "I stopped delivering babies a couple of years ago. but I still work with infertility patients. I work with a lot of menopausal patients, and I still teach. And I probably won ever stop doing that." In her spare time, she enjoys outdoor activities, woodworking, and golf.In 2003 Coney returned to her alma mater at the University of Mississippi to deliver the commencement address for the Medical Center graduates that year. She is its first alumnus to head a U.S. medical school, and she is proud that it is a historically black institution she leads. "Meharry has a very distinguished history, in that it has for a long time had a single mission that it has adhered to--and thats training minority health professionals," she told CBB. "I wanted to be a part of doing that, which I think is critical in shaping the future of medicine, particularly for minority populations in this country. And this gave me an opportunity to do that, one that I didnt have in majority institutions. So thats really what attracted me here. And thats what keeps me here."

Dr. Tulsiani received a Bachelor of Science degree from Ewing Christian College in Allahabad, India in 1962. He received a Master of Science degree in 1964 and PhD in biochemistry in 1968 from the University of Allahabad. He joined the research group of Professor Carubelli in 1968 as a post-doctoral fellow at the Oklahoma Medical Research Foundation. He was appointed as a research associate in Molecular Biology at Vanderbilt University in 1972 and joined Obstetrics and Gynecology in 1988. He was promoted to the rank of professor in 1996. Dr. Tulsiani served as director of Reproductive Biology Center Seminar series from 1991-1992, and was director of the Glycoprotein Core Facility of the Center for Reproductive Biology Research from 1992-1996. He has served as an ad hoc reviewer for the National Science Foundation since 1991. Dr. Roger Pepperell was born in 1942, graduated from the Melbourne University Medical School in 1965, subsequently passed Post-graduate Examinations in both OBGYN and Internal medicine in 1971 and completed a Research MD in Reproductive Endocrinology 3 years later. Following this he traveled to Glasgow where he spent 12 months working with Professor Ian Donald, the person who introduced ultrasound into clinical Obstetrics. He returned to the Department of Obstetrics and Gynecology at Melbourne University in 1975, and was promoted to Professor and Chairman of that Department in 1978, remaining in those positions until the end of 1998. From then until 2004 he was responsible for assessing the medico-legal problems and risk management at the Royal Women’s Hospital in Melbourne and other similar OBGYN institutions in Australia, but also continued his clinical and teaching duties. He officially retired from the University on 31/12/2003, but continued to see patients in antenatal outpatient clinics at the Royal Women’s Hospital and do an elective Caesarean section list and labour ward duty at that hospital until he left in November 2009 to take up a 2-year position as Professor of Obstetrics and Gynecology at Penang Medical College in Malaysia. Since 1975 he has been heavily involved in educational and assessment activities at the Royal Australian and New Zealand College of Obstetrics and Gynecology (RANZCOG), Melbourne University and the Australian Medical Council (AMC). He was a member of Council of the RANZCOG between 1986-1994, Honorary Secretary between 1992-1994, a member of the Board of Examiners between 1984 -1993 and Chairman of this Board of Examiners from 1989-1992. This Board is responsible for the assessment of all candidates who have completed their OBGYN specialist and subspecialty training in Australia. Since 1982 he has also been a member of the Board of Examiners of the AMC, the body responsible for the assessment of all overseas trained doctors who wish to practice medicine in Australia. In 1984 he was appointed as the Senior Examiner in Obstetrics and Gynecology, became Deputy Chairman of the full Board in 1998 and Chairman of the Board in 2002, a position he relinquished at the end of 2006. From then until he resigned in 2009 to take up the position in Penang, he was Chairman of the Clinical Examination Board of the AMC. His research interests have been diverse, predominantly in Reproductive Endocrinology and Infertility, but also encompassing other aspects of Obstetrics and Gynecology. Overall he has made 112 invited presentations to local and overseas meetings, has 100 publications in refereed journals, and has written 26 book chapters. In addition he has been heavily involved in publications of the AMC, including the book on “Anthology of Medical Conditions”, two handbooks on Multiple Choice Questions, and the recently published book giving detailed information about the Structured Clinical Exam.

Dr. S Joseph Huang obtained his MD degree from National YangMing University Taipei Taiwan and was trained as a surgeon then an obstetrician before he started his PhD study in physiology in the department of Cell and Molecular Physiology at the University of North CarolinaChapel Hill. Dr. Huangs broad interests include studying the role of decidual innate immunity under the influence of decidual stromal cells during the pathogenesis of preeclampsia using in situ in vitro and in vivo models.

During the pathogenesis of preeclampsia using primary leukocyte-free first trimester decidual cell and mouse as models. He demonstrated a marked excess of macrophages and dendritic cells as well as their recruiting chemokines and activating colony-stimulating factors (CSFs) in preeclamptic decidua. He also found that the pro-inflammatory cytokines, TNF-α and IL-1β, profoundly enhanced expression of an array of monocyte/macrophage- and dendritic cell-recruiting chemokines as well as CSFs. Currently, he is using a mouse model to scrutinize the role of macrophage, dendritic cells and their recruiting chemokines in the pathogenesis of preeclampsia. Dr. Huang is also interested in the pathophysiology of endometriosis. In collaboration with Dr. Lockwood, he is examining the expression of tissue factor in various immune cells in endometriotic patients. Projects in his lab are open and collaborative, with an emphasis on translating new basic knowledge into the clinic.

Two main lines of research are currently the main focus of my laboratory. The first is the role of dietary intervention, specifically phytoestrogens, on breast cancer risk. Biography :

Dr. Erma Z. Drobnis is Andrology Laboratory Director at the University of Missouri-Columbia (MU) School of Medicine and Clinical Assistant Professor in Ob/Gyn. She has studied sperm biology and reproductive medicine in diverse species for more than 30 years. After completing her undergraduate degree in mathematics, she finished her PhD in Physiology at the University of California, Davis (UCD), with advisor Dr. David Katz, studying the biophysics of sperm-oocyte interaction. Dr. Drobnis accepted her current position in 1994, achieving HCLD board certification that year. Dr. Drobnis has published many peer-reviewed and review articles on clinical Andrology and sperm biology. Current research interests include reproductive health in men from urban areas compared with rural areas with pesticide exposure, and the glycobiology of sperm. She has taught in the problem-based learning (PBL) curriculum in the School of Medicine at MU for 15 years and provides statistical assistance to Ob/Gyn Residents and faculty.

Biography :

Dr. Breton Barrier is OB/GYN Residency Program Director and Graduate Medical Education Committee Chair at the University of Missouri. He completed Medical School at University of Texas Health Science Center San Antonio, and OB/GYN residency at University of Missouri. In addition to practicing general obstetrics and gynecology, Dr. Barrier is actively involved in clinical and basic science research and graduate medical education. His research interests include placental immunology, maternal immune tolerance, primate models for endometriosis research, immunology of endometriosis, improving obstetrical outcomes and improving the effectiveness and safety of graduate medical education. Dr. Barrier serves as an Advisor for the Endometriosis Association, and is member and past Chair of the Reproductive Immunology Special Interest Group of the American Society for Reproductive Medicine. He is a Fellow of the American College of Obstetricians and Gynecologists and a member of the Society of Laparoendoscopic Surgeons and the Alpha Omega Alpha Medical Honor Society.

Dr.  Mira Aubuchon grew up in Wisconsin and then attended medical school at Northwestern University Medical School in Chicago. There, she became interested in reproductive endocrinology/infertility during my obstetrics and gynecology rotation. She then went to Washington University in St Louis for residency in obstetrics and gynecology, where she became interested in polycystic ovary syndrome (pcos) through collaboration with a neurologist investigating obstructive sleep apnea in that population. From there she completed a reproductive endocrinology/infertility fellowship at the Albert Einstein College of Medicine in the Bronx, NY and subsequently became board-certified in the subspecialty. She served as an Assistant Professor for 4 years at the University of Cincinnati College of Medicine in Ohio, followed by my current position as Assistant Professor and Director of the in-vitro fertilization program at the University of Missouri School of Medicine.

Dr. Premal H. Thaker did her fellowship training at M.D. Anderson Cancer Center and then joined the faculty at Washington University School of Medicine. She is currently in transition to the University of Pittsburgh-Magee Women’s Hospital as an associate professor in gynecologic oncology. Her research interests are in novel therapeutics for all gynecologic cancers but especially for ovarian and endometrial cancer. She is extensively published in the field of gynecologic oncology. Additionally, she has NIH funding as well as being a past recipient of a Gynecologic Cancer Foundation Grant.

Research Interest :

My laboratory is interested in studying abnormalities of protein degradation pathways in breast and ovarian cancer. The Ubiquitin-Proteasome-System (UPS) is responsible for degradation of over 90% of short-lived intracellular proteins. Protein degradation through Ubiquitin-Proteasome-System is a multistep process that begins with de-ubiquitination of ubiquitin-tagged target molecules by de-ubiquitinating enzymes following their entrance in the 20S catalytic chamber of the proteasomes were the actual degradation occurs. The polypeptide targets of the proteasome include proteins involved in cell cycle progression, survival and inflammation and while the ubiquitin-dependent proteasomal degradation is crucial for both normal and malignant cells the higher demand for metabolic/catabolic activity associated with the malignant phenotype renders the ubiquitin-proteasome pathway a suitable tool for cancer treatment. My laboratory is particularly interested in studying the role played by proteasomal- and lysosomal-assisted protein degradation pathways during the development and the progression of breast and ovarian cancer and in the development of new small-molecules inhibitors of ubiquitin-proteasome-system for targeting breast and ovarian cancer cells.

She has been involved actively in increasing the awareness of women towards cancers, osteoporosis, etc. She has been the Editor of Ahalia Medical News.

Currently, she is working as a Consultant in the Department of Gynae Surgical Oncology at Galaxy Cancer Institute, Pushpanjali Crosslay Hospital, Delhi-NCR, India.

The focus of her practice is Gynecology and Gynecologic Oncology exclusively. She does not practice Obstetrics. She is pursuing Postgraduate Diploma in Medico legal Systems (PGDMLS). Antonio Simone Laganà was born in Reggio Calabria (Italy) on 8th May 1986. During his university career he was trained in particular in histology and embryology (supervisor Prof. Domenico Puzzolo), Clinical Pharmacology (supervisor Prof. Edoardo Spina), Immunology and General Pathology (supervisors Prof. Vincenza Sofo, Dr. Francesca Maria Salmeri) and finally in Obstetrics and Gynecology. He attended also the Department of Interventional Radiology of the Virginia Commonwealth University (Richmond, Virginia - USA), where he studied technique of uterine fibroids chemioembolization (supervisor Prof. Uma Prasad), and the Department of Obstetrics and Gynecology of the “Klinicky Centar” (Belgrade - Serbia. Supervisor Prof. Milan Terzic). He earned his graduation at University of Messina (Italy) and became Resident Medical Doctor in Obstetrics and Gynecology. He is “In-Training Member” of the Society for Gynecologic Investigation, Regular Member of Italian Society of Perinatal Medicine, and of Giorgio Pardi Foundation’s professional research team. Currently, he is attending a Master of “gynaecological minimally invasive and robotic surgery” (supervisor Prof. Vito Cela, University of Pisa).
