Antiretroviral therapy was implemented after the announcement of the Operational Plan for Comprehensive HIV and AIDS Care and Treatment for the public health system with treatment regimens specified. The ART roll-out has profoundly reduced AIDSrelated morbidity and mortality in resource poor settings. The appropriate use of HIV drug-resistance testing together with improved quality of care in HIV programmes will ultimately result in the long term reduction of the spread and emergence of drug resistance in low income settings.
