A 9 year old boy was referred to me with effort angina (NYHA class 3). He had familial hyperlipidemia and was on statins. He had a strong family history of coronary artery disease from the maternal side, where his mum, two uncles and aunt all had bypass surgeries in their twenties. His coronary angiogram showed a dominant right coronary artery with an 80% ostial lesion and a 75% ostiel LAD lesion that were difficult to be stented by his treating cardiologist. His examination was unremarkable except for huge xanthomas over his elbows, knees and ankles. His cardiac echo showed a normal ventricle and valves. I operated on him and he had a left internal mammary to his LAD, and a right internal mammary to his RCA. Two year follow up showed him to be asymptomatic, with a tight lipid control. There are a few similar cases reported in literature, but this is one of the youngest to have a bilateral mammary surgery.
Yanal F Al-Naser is a Consultant Cardiac Surgeon from Amman-Jordan. He graduated from the University of Jordan in 1999, and was awarded the Jordanian Boards in General Surgery (2004) and cardiac surgery (2009). He was trained in Manchester – England as an Adult Cardiac Surgeon and finished his training in 2012. He became a consultant adult cardiac surgeon in 2012 at Queen Alia Heart Institute in Jordan. He has a number of publications, and is a member of the Jordanian Cardiac Society and the Royal College of Surgeons in the UK.
