A Nursing care plan is a formal process that includes correctly identifying existing needs as well as identifying potential needs or risks. Without the nursing care planning process, quality and constancy inpatient care would be lost. Care plans also provide a means of communication among nurses, their patients, and other healthcare outcomes. A care plan flows from each patient’s unique list of diagnoses and should be organized by the individual’s specific requirements. They assist as a guide for conveying staff to care for the client.
