    Clinical history and physical examination—24-hour blood pressure (BP) monitor, measurement of orthostatic BP after standing for 3 minutes or home monitoring of orthostatic BP will help in making the diagnosis.To test the hypothesis that delayed OH is an early and milder form of OH that progresses over time, the investigators reviewed the medical records of 230 previously reported patients who completed autonomic testing at Beth Israel Deaconess Medical Center in 2002 and 2003. All information on clinical diagnosis, mortality, medication use, and autonomic testing was included. Complete follow-up data was available in 45 patients with delayed OH, 42 with OH, and 75 controls, reported the investigators. The review showed that 37% of patients with delayed OH developed an alpha-synucleinopathy.
