Abstract

Chronic heart failure (CHF) is a complex and progressive condition that impacts millions of individuals worldwide, leading to significant morbidity, mortality, and healthcare resource utilization. Traditional management strategies, such as medication optimization and in-hospital rehabilitation, have been the mainstay of treatment; however, the rise of telemedicine has introduced new possibilities for enhancing patient care.
