Introduction: Asian race have high prevalence rate of congenital entropion. Old report related that over 27% of Japanese children have congenital entropion at 1year old. One of the structural cause is development of epiblepharon which tract the lower eyelid to the upper eyelid, is also highly existed in Asian. However, the designing of epicanthus flap is relatively difficult, epiblepharon-plasty is not popular procedure in Japan.This time, we developedthe easy designing method, thenreport the surgical method and the result. Cases: From Jan 2010 to Aug 2011, one surgeon performed congenital entropion surgery on 28 cases. We analyzed this series of 28 cases retrospectively, which consisted of 17 females and 11males, average age was 7.6 years old. Ten patients required epiblepharonplasty with lower lid procedure and 18 patients required only lower lid procedure. Surgical Method: On the epiblepharon, about 2mm-wide small triangle shape �fish-tail� flap was designed continuously �fish-body� marking on subciliary lower eyelid. After incision, Hotz procedure was done and skin flap was sutured edge to edge. Result: Triangle skin flap diminished the tension of orbicularis to the superior direction. Minimally after 6 months, shape of medial canthus was opened widely, and the direction of cilia was stable. Conclusion: This plasty is easy to design following Hotz procedure, and to correct congenital entropion effectively. To recognize the shape of congenital entropion accompanied by epiblepharon isimportant to treat them basically.

Kazuaki Nakauchi is a Japanese Ophthalmologist, his specialty is oculoplastic. He has worked as ophthalmologist since 1996, and has completed his Ph.D at the age of 36 from Osaka University by the research of artificial retina. He was a fellow at Singapore National Eye Center at 2009. Now, he is working as an assistant professor at Hyogo College of Medicine as an oculoplastic surgeon. He is also a good father of 4 children.
