Abstract:

The present study deals with intervention in drug therapy and Adverse Drug Reaction monitoring by the clinical pharmacist. Over a period of time the role of clinical pharmacist has evolved from mere dispensing to patient drug therapy management, which has resulted in better disease management and cost savings to the patient. Clinical pharmacist primary job is to interact with the health care team, interview and assess patients, make specific therapeutic recommendations, monitor patient responses to drug therapy and provide medicines information. The present research work involves documentation of medication errors, drug-drug interactions, drug-food interactions and detection and monitoring of Adverse Drug Reactions. The interventions were discussed with physicians and suitable suggestions were provided. Pharmacist also answered queries and carried out-patient counseling. Data was obtained by reviewing patients medication charts of prospective series of patients admitted in the medicine wards of St. Martha?s Hospital. A total of 210 medication charts were reviewed from July 2010 till date. Out of which 20 Adverse Drug Reactions were found out of which one was severe and remaining were mild to moderate. Medication errors included 3 inappropriate indications, 1 same class of drug, 1 drug interaction, a total of 10 queries were answered to the physicians and 6 patient counseling were carried out.

Rafi Md has completed Master in Pharmacy in department of Pharmacy Practice in Al-Ameen College of Pharmacy, Bangalore. Currently working as Senior Drug Safety Associate in Pharmacovigilance unit in Quintiles Technologies India (P) Limited, Bangalore.
