Abstract :

Clear cell carcinoma in parotid gland is uncommon and its extension through the jugular foramen to present as a primary intracranial SOL is extremely rare. We report a case of middle aged female presenting with cerebellar symptoms and identified as having a CP angle tumor on MRI with intra and extra cranial components extending across the jugular foramen. The intracranial part of tumor was diagnosed on biopsy as a metastatic clear cell carcinoma. Four months later the patient came again with complaints of dysphagia, voice change and an ill-defined tumor mass in parotid region with a bulge in oropharynx. Diagnosed as â??Clear cell carcinoma of Parotid glandâ?? on biopsy. The history of intracranial tumor, correlated only with retrospective clinical study. The histology was identical with that of the intracranial mass. Markers Pancytokeratin, EMA were positive and other markers were negative on both biopsies. Metastatic lesions in jugular foramen are destructive and infiltrative. CECT of this tumor showed a welldefined, mildly enhancing mass extending through it. The final report signed out was â??Clear cell carcinoma NOS of parotid, with intracranial extension through jugular foramenâ?. A low-grade malignant tumor that choses oral cavity, is rare in parotid. Present as â??swelling onlyâ?? in many, but with pain / ulceration in some. The duration of these tumors vary from one month to 15 years. This rare parotid tumor, with intracranial extension through Jugular foramen probably is the first report in literature and should be considered in differential diagnosis for tumors present similarly.

Graduate in MBBS, diploma in clinical pathology and postgraduate in M.D Pathology from Guntur Medical College, AP, India. Worked in Medical colleges in the capacity of assistant professor, associate professor and professor & Head of the department of Pathology from 1981 till date. Having experience as both undergraduate and post graduate teacher in Pathology for >25 years and as diagnostician in various branches of Pathology; attended many conferences to present papers or as a judge; has 17 publications in various national and international journals. Member of Indian association of Pathologists and microbiologists and a consultant Pathologist for 2 decades for a local super specialty hospital; conducted zonal CMEs and seminars; Presently working as professor and head of the department of Pathology; NRI medical college; AP; India; where there is annual intake of 150 Under graduate students and 12 Post graduate students in Pathology in 3 years of training. The labs are in quality control under NABL (National board)
