Current status of preoperative education for Japanese esophageal cancer patients

Abstract :

Background: As esophageal cancer surgery is highly invasive, postoperative complications occur readily, and there are high rates of mortality. Therefore, in terms of nursing care, there is a demand for preoperative education that is effective in preventing postoperative complications. Objective: A literature search was carried out to clarify the current status of preoperative education for preventing complications after esophageal cancer surgery. Methods: Using Ichushi-Web, Japanese publications between 2000 and 2016 were screened for as follows: 1. Original reports were searched for with the key words "esophageal cancer" "respiratory complications" "prevention" and those describing to esophageal cancer patients who underwent surgery that was not of low invasiveness were analyzed. 2. All publications were searched for using the key words "perioperative period" "esophageal cancer or esophageal tumor" "postoperative complications" "patient education". Of the publications found, those describing preoperative education preventing surgical complications, irrespective of the type of postoperative complications and mode of surgery were analyzed. Results & Discussion: Three reports found in search 1, described education provided by physiotherapists rather than nurses, who carried out combinations of multiple instructions and were involved in the patients continuously from before to after surgery. Many of the instructions comprised methods of which the patients were able to work on by themselves. Three original reports and two commentaries/reviews were found in search 2. The preoperative education was planned and provided based on multidisciplinary liaison, in which some institutions established the specialist teams. Comprehension checks were provided after the preoperative education to achieve a firm grasp of the information, and such involvement was continued after surgery. The results showed the need of the establishment of the support system in the nursing care, which helps the patients to work on the education by themselves from preoperative out-patient visits throughout the postoperative hospitalization period.

Biography :

Yuuka Akuzawa is an Assistant Professor at Juntendo University and she has 10 years clinical experience, as well as an undergraduate education in Adult Nursing including Perioperative Nursing and Emergency Nursing. Her main research topics are complementary medicine using the aromatherapy, improving of the quality of the perioperative nursing. Much of her work has been on education for perioperative nursing and perioperative education for patients using ICT teaching materials.
