Metabolic Syndrome (MS), especially the lipodystrophy syndrome (SL) in people with human immunodeficiency virus (HIV) is characterized by metabolic abnormalities, insulin resistance (IR), abnormal redistribution of body fat, functional disorders of many organs and autonomic systems (dysautonomia) in addition to increased risk of cardiovascular disease (CVD). This study aimed to perform a systematic review on the Metabolic Syndrome/ Lipodystrophy Syndrome and the risk of cardiovascular disease in people with HIV / AIDS. We searched CENTRAL and DARE (Cochrane Libray issue 10, 2012) MEDLINE (1966 to 20-11-12), EMBASE (1980 to 2012 week 30), and 8 other databases, up to november 2012. We also searched relevant websites, reference lists and forward citation tracking of included studies, and handsearched six key journals. We observed that the absolute risk of CVD in this population with insulin resistance and diabetes mellitos (DM) type I and II, is dramatically higher as compared to nondiabetic individuals at the same age. The classic risk factors for the presence of CVD only partially explain these results. The presence of IR has been described in subjects with MS and HIV / AIDS and may contribute to the increased risk of cardiovascular disease found in this population
