Abstract
Rejected microbiology investigations for pediatric patients, due to clinical incidents on the pediatric ward, an audit was carried out to determine whether rejected microbiology investigations could have affected a child’s diagnosis or management plan and to see if they could have been avoided. A list of rejected specimens was obtained from management for a two month period. The electronic system was used to collect information on the diagnosis, management and subsequent investigations (clinical audit 1). Results were presented to staff; posters and notes were created and placed in clinical areas and in the new starters information pack. After which the audit was repeated (clinical audit 2). Clinical audit 1: There were 44 rejected specimens (39 patients), 42 of these were avoidable e.g. wrong container was used, incomplete form or insufficient amount. If these tests were to give a positive result the diagnosis would have been different for 7 patients and the management plan amended in 25 patients. Seven tests were subsequently repeated and after discharge 3 patients re-attended with the same presentation. Clinical audit 2: There were 22 rejected specimens (17 patients), 21 of these were avoidable. A positive result would have changed the diagnosis in 3 patients, and altered the management of 9 patients. Educating staff on how to collect specimens correctly and the potential detrimental impact of not doing so, has led to a 50% reduction in rejections. In total 95% of rejections were avoidable, based on the reasons given, human factors play a role. Working to minimise rejections can reduce the number of patient safety incidents while saving the department money.
