Abstract :

With the disruption in healthcare, hospitals are moving patients down the continuum either into skilled nursing facilities or back home with home care services. These efforts place new burdens on the post-acute network to avoid readmissions, improve outcomes and provide a high level of patient satisfaction. An effective strategy incorporates telemedicine, remote monitoring and sensor technologies to address unnecessary hospital readmissions. Unfortunately current payment systems have not moved at the same pace as the current technology. One option utilizes healthcare call centers staffed with physicians and other healthcare professionals to provide cost effective models to respond to this new type of healthcare information.
