Abstract
The aim of this study is to analyze our experience in treatment of patients with diabetic Charcot's arthropathy complicated with purulent infection. We have analyzed 37 patients after Ilizarov method treatment aged from 41 to 73 with diabetic Charcot's arthropathy complicated with infection. According to Brodsky classification we observed: Type 1, 6 patients; Type 2, 7 patients; Type 3A, 14 patients; and Type 4, 10 patients. Treatment consisted of two stages: at the first stage we performed radical sequestronecroctomy and fixed foot using Ilizarov frame in favorable for healing position, antibiotics up to 3 weeks; at the second stage we performed reconstructive restorative surgeries in order to fill in bone defect. Along with this treatment patients received adequate insulin therapy and therapy to decrease neuro-ischemic processes in tissues. Short terms follow- up (1 month) and long term follow up (1 to 6 years) was done after frame removal. One patient after 8 months of treatment had leg gangrene. Amputation was done and as a result the infection was eliminated, and support ability of limb was restored.
