Abstract We report on a 44-year-old smoking man with sarcoidosis stage III presenting with bilateral spontaneous pneumothorax. The diagnosis was confirmed by biopsies from the lung parenchyma, visceral pleura and bronchial mucosa. Bilateral pleurectomy was needed since long-term tube drainage turned out to be unsuccessful.
