Abstract Few cases of spontaneous rupture of the tibialis anterior tendon have been reported in the literature. We report a case of a 66-year-old male with a spontaneous rupture of his left tibialis anterior tendon, with a delayed presentation of approximately 3 months. At the time of reconstruction there was marked retraction of the proximal portion of the tibialis anterior tendon, which precluded an end-to-end repair; therefore, we used an Achilles tendon allograft.
