Abstract ISSUE: A paradigm shift is afoot in which the "last shall be first," namely, use of long-acting injectable (LAI) antipsychotics, rather than being reserved for use only at the last stages of schizophrenia, may be shifting to first-line treatment of early episodes of this illness.
