Years ago, hepatocellular carcinoma (HCC) was known as a major cancer killer but did not then garner much attention in developed countries. This paradox was because most cases occurred in Asia and sub-Saharan Africa, and also because early diagnosis was often not feasible. Surveillance programmes for early detection were not in place and advanced imaging was unavailable. Hence HCC was diagnosed at advanced stages for which effective therapies were lacking. This situation changed over the past three decades because of major achievements in diagnosis and treatment, and here we update our Lancet Seminar.
