Abstract The osteogenic exostosis of the ribs are unfrequent. Their location on the first rib is exceptional. One case is described. The authors, after a review of the literature, recall the particular symptomatology which can be observed, like thoracic outlet syndrome. A partial or total resection of the first rib is advocated. The surgical approach has to be discussed.
