Towards developing a scoring system for febrile thrombocytopenia

Abstract:

A retrospective study spanning over a period of four months from June 2010-September 2010 was carried out in Rajiv Gandhi Medical College, which is a central civic hospital and teaching institute in Thane, Maharashtra, India. A total of 93 subjects admitted to the medical ward or ICCU with fever and thrombocytopenia defined as a platelet count of <150?109/L were included in the study. Evaluation: Records of vital signs of the patients at time of admission were noted. Clinical examination findings of patients with respect to neurological status, respiratory, renal, haematological and other complications were noted. Routine investigations such as CBC, glucose, urea, serum creatinine, bicarbonate, serum electrolytes, and LFT were noted. In some of the patients cause of fever was found and tabulated accordingly. The study protocol was approved by the Institutional Human Ethical committee (IHEc) of Rajiv Gandhi Medical College. Early warning score was prepared on the basis of following points: Statistical analysis: Statistical analysis was performed using the statistical software package SPSS (version 20). The nonparametric chi-square test and t-test were also applied in comparative analysis results between different groups and to find significance (p) value. Inclusion criteria: All patients >18 years presenting with fever and thrombocytopenia with platelet count of <150?109/L. Exclusion Criteria: Patients younger than 18 years, pregnant patients, patients on drugs causing thrombocytopenia (chemotherapy/ immunosuppressants/antiplatelets), previously diagnosed patients with chronic thrombocytopenia due to any cause.

Shaylika Chauhan has completed her Bachelors in Medicine and Bachelors in Surgery (MBBS) from Pravara Institute of Medical Sciences, India and her Internal Medicine residency training under National Boards of Examinations, New Delhi, India (Board Eligible). She is also ECFMG certified. She has published 2 papers in reputed journals and is working in the field of Internal Medicine for 6 years.
