Patients are usually asymptomatic and diagnosed accidentally. It can occur from newborn to 10 years with a median age of 10 months. The tumor is mainly cystic but sometimes it will turn into a solid component in rare cases. Diagnosis of large cystic hepatic masses is often difficult in children.

Cystic mesenchymal hamartoma of the liver can be distinguished from other liver masses such as infantile haemangioendothelioma, hydatid cyst, biliary rhabdomyosarcoma and hepatoblastoma which can be considerable overlap in their radiological appearance. Cyst hydatid is a common parasitic disease frequently involving the liver that is endemic in certain parts of the world.
