Vancomycin

Vancomycin, a glycopeptide, has long been considered a time-dependent killing antibiotic, and therefore therapeutic drug concentration monitoring by using peak concentrations, troughs, or both, has been practiced for years. Therapeutic drug monitoring for vancomycin has also been largely debated for the same amount of time. Current consensus for treatment of serious S. aureus infections is that vancomycin is a slowly-cidal, time-dependent killing antibiotic However, AUC/MIC, not T>MIC, is the most predictive pharmacodynamic parameter.
