Disease management is a co-ordinated health management system to deal with chronic health disorders and health problems. This makes patients aware of how to manage the diseased state and how to prevent exacerbation of the disease. The major aim of the system is to improve the quality of health care, reduce hospitalization risks, spreading of disease in case of epidemics and expense for the treatment. The complete eradication of lots of diseases was possible by efficient disease management systems. Different stages of disease management system includes identification of a effected target population, collaboration with physicians and service providers, awareness to patient about self-management and finally analyzing the result of process outcome. This self-management system helps the patients to make behavioral changes and effective health care interventions and preventive measures to fight against the chronic illness.
