Alabama Primary Health Care Association (APHCA)

     Alabama Primary Health Care Association mission is to strengthen Alabama's health center network through leadership, collaboration and the coordinated services that are available to all Alabamians and fosters improved health outcomes. Community health centers reliable on focusing the importance of the health of individual patients have strived to improve the economic status of the entire community. This community oriented health care differs from the traditional health care providers in several ways. Through high quality, cost efficient care, it mainly reduces health disparities, improves birth outcomes, effectively manages the chronic disease and mainly stimulates economic growth in the community. Through a one-stop primary care model, medical aid, followed by the services designed to ensure the alterations of the medical care provider such as clinical and health education and nutritional management is the fundamental key to success. It mainly specifies the importance of CHCs stating that the model has effective in increasing the access to care but also helps in improving health outcomes for the higher-risk populations they serve.
