Laser cordectomy in glottic carcinoma

Abstract
Endolaryngeal extirpation of vocal cord cancers is a controversial issue over many decades. Transoral excision of vocal cord cancer was reported by: Lynch (1920), New & Dorton (1941), Mc Cabe & Magielski (1960), Kleinsasser (1968), Littlie & DeSanto (1973) and Le Jeune (1977). CO2 laser excision of vocal cord cancer was described by Jako et al (1978):in the form of excisonal biopsy of suspected lesions and excision of rec. VC cancer after radiation. Vaughan et al (1978) mentioned 4 indications for CO2 laser surgery in laryngeal cancer: � Diagnostic: bulky & post irradiation recurrence � Debulking: reduction of tumour mass � Airway re-establishment: Avoid tracheotomy � Therapeutic: CO2 laser Cordectomy In this presentation the different types of laser cordectomy including superficial, partial, total and extended cordectomy will be illustrated with the indication of each type (Steiner). Also the European Laryngological Society Classification of laser cordectomy will be demonstrated (Remacle). The basic requirements, limiting parameters and surgical strategy including patient and tumor selection will be evaluated through my personal experience in the last 20 year.
