Abstract
The incidence of differentiated thyroid cancer (DTC) is increasing. The classical concept of treatment of DTC: Total thyroidectomy plus radioiodine therapy is quickly changing. The new trend is to adopt a tailored treatment based on the individualized risk assessment. This assessment takes into account some special issues like the risk of death, the risk of recurrence and the risk of persistent disease among others. Today the debate is largely centered on the extent of the disease and the risks of each surgical approach and divergent recommendations existing for the optimal surgical resection among treatment guidelines and consensus statements. It is very important to point that even the incidence of micropapillary carcinoma has increasing, tumors bigger than one centimeter are also much more frequent nowadays. This way the treatment could vary from an active surveillance to a total thyroidectomy plus prophylactic neck dissection. The purpose of my presentation is to show an objective way for the new trends in surgical treatment of DTC.
